What does the program offer?
It provides assistance and supportive services to women and families in Washington County with a substance use disorder or a co-occurring mental health and substance use disorder. Peer Recovery Support and case management focuses on a collaborative approach in developing an individualized care plan to:
- Identify specific goals
- Support relapse prevention
- Offer crisis management
- Provide referrals to community resources
- Collaborate with care providers
- Utilize trauma-informed care and evidence-based practices.
Eligibility requirements: Pregnant/postpartum women and women with children, located in Washington County, identifying a substance use disorder or co-occurring mental health and substance use disorder.
How to participate/enroll: Call 240-313-3310. Walk-ins are welcome if you cannot call.
